To NOMINATE
a Brave Heart please click on the application form on the right,
print it out, fill it in and send it to the address below.
A nominated child must reside within the area of benefit (see Scope page of this website) and be between 5 years of age and under the age of 16 years on the 1st day of January in the year of award.
If you do not have access to a printer, simply write us a letter
telling us all about your Brave Heart and that will do just as well.
